Please read the ordering instructions then click on purchase orders.  When the purchase order pops up, right click on it and click print.  Fill in the purchase order and send it with your check or money order to the address below.

Ordering Instructions:

 

Please mail all orders to: Joe Loszynski, 4 Dorlon Drive, Queensbury, New York 12804.  If you have any questions about your order, please call Joe Loszynski at 518-307-9448. It is imperative that you provide the name of the item, whether adult or youth, size, color and the quantity. If you prefer to place the order by phone or have questions concerning any order, please call Joe at 518-307-9448.  All Orders need to be mailed to Joe so appropriate documentation exists on each order.

 

All checks and money orders are to be made payable to the Lake Hadlock Association and mailed to Joe Loszynski, 4 Dorlon Drive, Queensbury, New York 12804. Do not send cash via US Mail. Kindly be advised that no orders will be placed with the vendor until the funds are received by Joe.

 

SHIPPING

 When your order is complete, Joe will inform you when you can pick it up.  

 If you would like your order shipped, please contact Joe prior to mailing your order and check to him so that shipping charges may be determined.

*** Please note that all orders will take two weeks to be filled from the date the order is placed with the vendor. If orders are intended as gifts for Christmas, it is recommended that the orders be placed well in advance to prevent a late delivery due to high volume shipping times, etc.
